Social Worker – Mental Health Navigation and Social Work (2 Positions)
This is a blended Social Worker-Mental Health Navigator role responsible for connecting patients to appropriate services through screening, assessment, and following referral pathways. This role also assists patients who require support for finances, employment and housing. This full-scope position works collaboratively with an interdisciplinary team of physicians, nurses, dietitians, pharmacists, kinesiologists, behavioural health consultants, mental health therapists and psychiatrists to provide patient support.
Behavioural Health Consultant (BHC)
Join our mental health team in the role of Behavioural Health Consultant (BHC). This role supports our mental health referrals working to the full scope of practice. The Behavioural Health Consultant works collaboratively with patients, primary care physicians, interdisciplinary team and other healthcare and community partners to provide comprehensive evidence-based care.
Quality Improvement Facilitator
The Quality Improvement Facilitator assists clinical teams to increase efficiency, access, and clinical care delivery aligned within the Patient-Centered Medical Home model. This will include, but is not limited to, assessing/analyzing current processes and effectiveness, identifying opportunities for improvement, developing customized work processes for enhanced patient centered care and more.
Registered Nurse – Chronic Disease Management (CDM) (Part-Time, 1 yr Temporary)
The EOPCN is recruiting a Registered Nurse (RN) – Chronic Disease Management (CDM) to work at our central site. The RN practices in a primary care clinic setting with physicians and other healthcare professionals as a member of a multidisciplinary care team, and works with patients (and their identified care partners) suffering from or at risk of chronic conditions such as diabetes, hypertension, asthma, COPD, cancer, and arthritis.
