Licensed Practical Nurse (LPN) – Foot & Lower Leg Clinic

The EOPCN is recruiting a certified Advanced Foot Care LPN.

Reporting to the Program Manager, Clinical Director or designate, the Licensed Practical Nurse (LPN), supports the delivery of Primary Care Services in the Footcare and Lower Leg Clinic, Edmonton O-day’min Primary Care Network (EOPCN) central location. The LPN uses the nursing process as a member of, and in collaboration with, the multidisciplinary team, in the delivery of consistent quality care to meet the needs of the patient.

The LPN provides restricted activities and care within the full scope of practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). The LPN accepts responsibility for continuing competency through ongoing professional development including participation in education programs, research, and quality activities. The LPN adheres to CLPNA professional practice standards, guidelines and competencies, and Edmonton O-day’min Primary Care Network (EOPCN) policies and procedures. The LPN is responsible and accountable for their own practice as defined by CLPNA. The LPN practices in a setting where patient care needs are well defined and established, and where changes in the health condition of the patient are predictable.

 

KEY RESPONSIBILITIES & DUTIES

• Provides evidence-based footcare nursing within the scope of LPN practice.
• Performs clinical procedures including but not limited to microfilament testing, ABIs/TBIs, skincare and footcare, toenail care using instruments such as clippers and dremel.
• Performs patient-centered foot and lower leg health assessments, including but not limited to medical history and medications, physical and psychological health, patient knowledge, and skills pre and post intervention, patient living/working environment, supports available, and functional status.
• Plans interventions based on the most current evidenced-based guidelines and best practices such as: AHS Diabetes footcare clinical pathway, AHS Lower Leg Edema Management Clinical Resource, Inslow’s Diabetic Foot Screening Tool, etc.
• Evaluates and suggests modifications to patient-centered action based on patient’s on-going needs.
• Provide health teaching, addressing topics such as healthy lifestyle, risk-reduction behaviors, and preventative self-care.
• Obtain and update patient histories and medical information on the Electronic Medical Record (EMR), using appropriate evidence-based assessment techniques, tools, and instruments during patient visits, with the ability to pass this information onto the appropriate team member.
• Collects, documents (EOPCN EMR) and reports health information from objective and subjective sources to identify patient health needs, in accordance with legal, ethical, organizational, and professional requirements.
• Follows nursing process of assessment, nursing diagnosis, planning and intervention, and evaluation of outcomes. Monitors changes in health status using critical judgment skills.
• Observes, reports and documents change in patient status and deviations from anticipated response/outcome.
• Coordinate patient referrals to other disciplines, and community resources as needed.
• Adhere to best practices for cleaning, disinfection, and sterilization of equipment/instruments and the workplace environment.
• Keep current on best practices by updating self and team with new clinical information and guidelines as they change/develop.

 

QUALIFICATIONS:

• Graduate of an approved Licensed Practical Nurse program, accredited through CLPNA.
• Footcare Certification for Advanced Practice Footcare.
• Current registration with CLPNA.
• CPR C for Healthcare provider, ABI Certificate is an asset
• Experience in using electronic medical records and demonstration of proficient computer skills.
• A minimum of one-year primary care nursing experience is preferred.
• Lower Leg Assessment training, Infection Prevention, Sterilization and Autoclaving is an asset.
• Physically capable of performing duties related to position (i.e., assisting clients with mobility issues during clinic visits, frequent bending, lifting a variety of clinic equipment and tools.)
• Successful applicants will require a clear Police Information Check with Vulnerable Sector check.
