Primary Care / Mom Care Licensed Practical Nurse

Full-Time Permanent 1.0 FTE (37.5 hours/week)

Reporting to the Program Manager or Designate, the Primary Care LPN offers standardized, patient-centered care to support patients in the primary care environment. Their role is complementary to the primary care physician and other members of the multidisciplinary health care team. EOPCN practical nurses work to full scope, supporting the health and well-being of patients from a nursing care perspective. Specifically, the Primary Care LPN focuses on health assessment and practical nursing support for patients of all ages, with the goal of improving health outcomes and access to primary care services. 

The EOPCN Primary Care LPN practices safe, evidence-based care within a primary care clinic setting for clients prenatal to older adult with a variety of bio-psycho-social needs. As a member of the EOPCN co-located in a physician clinic, the LPN is also a representative of the EOPCN and a link to other EOPCN supports and services. 

A portion of this role will be dedicated to supporting low-risk prenatal care with the Mom Care Docs team at the Allin Clinic. The LPN will also provide support to EOPCN member clinics as organizational needs arise. 

KEY RESPONSIBILITIES & DUTIES

The Primary Care LPN focuses on assessment and skill-based interventions such as:   

  • Health assessment, screening, and documentation of health information from various sources to identify patient health needs in accordance with legal, ethical, organizational, and professional requirements.  
  • Uses critical thinking to assess, plan and evaluate nursing interventions that impact client outcomes.  
  • Performs clinical procedures (but not limited to) wound care, medication and immunization administration, ECG, 24h blood pressure monitoring, ear syringing, women’s health screening.  
  • Prepares examination rooms including selection, setup and maintenance of medical supplies and equipment for all examinations and procedures.  
  • Assists physicians with procedures.  
  • Provides basic healthy lifestyle education for patients including healthy behaviours, risk reduction and illness prevention.  
  • Participates in clinic quality improvement initiatives.  
  • Assist and support patient navigation through the healthcare system and community.  
  • Provides patient education related to prenatal health, including nutrition, physical activity, common pregnancy concerns, self-monitoring, and preparation for childbirth. 
  • Supports the Mom Care Docs team by coordinating patient information, ensuring timely documentation, and assisting with follow-up of prenatal care plans. 
  • Facilitates referrals and connects patients to appropriate prenatal, postpartum, and community support resources as needed. 
  • Assists physicians and the Mom Care Docs team with prenatal procedures, screening activities, and routine assessments within the LPN scope of practice. 

QUALIFICATIONS:

Candidates must meet the following qualifications:

  • Minimum of two years of nursing experience within a primary care or community health setting.
  • Graduation from an approved and accredited Licensed Practical Nurse program, with active registration in good standing with the CLPNA.
  • Experience in maternal health or women’s health settings is preferred.
  • Demonstrated competency in basic nursing care procedures.
  • Immunization and Women’s Health Certification.
  • Knowledge of perinatal standards and low-risk prenatal care guidelines.
  • Ability to identify psychosocial factors affecting pregnancy and incorporate this understanding into nursing care.
  • Current CPR certification (Heart and Stroke Foundation BLS Level C – Health Care Provider).
  • Excellent interpersonal skills with the ability to work collaboratively within a multidisciplinary team.
  • Strong communication, critical thinking, problem-solving, and organizational skills. Strong computer proficiency, including experience with electronic medical records and Microsoft Office applications.
  • Knowledge of client care models, with an emphasis on multidisciplinary, team-based approaches.
  • Proficiency in using Electronic Medical Record (EMR) systems such as Accuro.
  • Proficiency in additional languages such as French, Arabic, Tigrinya, or Oromo is considered an asset.
  • Access to reliable transportation for travel to various clinics and the EOPCN central office.
