Quality Improvement Facilitator (QIF)

(FTE Details) Permanent Full-Time 1.0 FTE (37.5 hours/week)

Reporting to the Medical Home Supervisor or designate, the Quality Improvement Facilitator (QIF) provides in-person practice improvement facilitation to Edmonton O-day’min Primary Care Network (EOPCN) member clinics. The role supports physicians and clinic teams to implement and sustain Patient’s Medical Home (PMH) principles through structured, data-informed improvement work.

KEY RESPONSIBILITIES & DUTIES

The Quality Improvement Facilitator (QIF) supports primary care clinic transformation through in-person practice facilitation within EOPCN member clinics. The role involves conducting and supporting the Medical Home Assessment (MHA) and Scorecard to assess clinic readiness, establish baselines, and measure progress over time. The QIF partners with physicians, clinic leadership, and interprofessional teams to identify improvement opportunities and implement Patient’s Medical Home–aligned initiatives related to panel management, continuity, access, team-based workflows, and care coordination. Using data and EMR-informed insights, the QIF coaches and mentors clinic teams to design, implement, and sustain new processes. The role also requires close collaboration with Medical Home team members and EOPCN stakeholders to ensure clinic-level improvement work aligns with broader program and system priorities.

 

QUALIFICATIONS:

Experience:

  • Minimum 5–10 years of progressive experience within a primary care or family medicine environment is required, including experience working directly with family physicians and interprofessional primary care teams.
  • Demonstrated in-clinic primary care experience is required, with a strong understanding of clinic workflows, team-based care, and day-to-day practice operations.
  • Experience as a coach, mentor, or facilitator supporting change and improvement within primary care clinics is strongly preferred.
  • Experience, knowledge, or formal training in change management, improvement methodologies, and quality improvement principles, with demonstrated application in primary care settings, is an asset (training will be supported).
  • Experience in public speaking, group facilitation, and presenting to clinical audiences is an asset.

Education:

  • Undergraduate degree or diploma in a related field of study, preferably within a health discipline.
  • A combination of relevant education and substantial primary care practice improvement experience will be considered.
  • Formal training in quality improvement, project management, or change management methodologies (e.g., Lean, Six Sigma, Prosci or equivalent) is strongly preferred.

Special Knowledge/Skills:

  • Demonstrated application of formal project management and change management methodologies in healthcare or primary care settings.
  • Strong understanding of stages of change, system change theory, and their practical application within primary care environments.
  • Proven ability to facilitate groups, lead structured improvement discussions, and support teams through change.
  • Experience working with Netcare and navigating provincial health information systems.
  • Experience working with multiple primary care EMR platforms and using EMR data to support improvement work.
  • Intermediate to advanced proficiency in Microsoft Office applications (Word, Excel, Outlook, Teams, and PowerPoint).

  • A valid driver’s license and access to a reliable vehicle
  • Recent (within 3 months) criminal record check/police information check (including vulnerable sector search) prior to the first day of hire.
