KEY RESPONSIBILITIES & DUTIES
The Quality Improvement Facilitator employs PCN supported improvement principles and tools to assist teams to increase efficiency, access, and clinical care delivery as indicated within the Patient-Centered Medical Home model. This will include, but is not limited to, assisting with understanding their current access for care, their processes for patient attachment, their model of care delivery, and the most effective use of Electronic Medical Records to support service delivery improvements. The Facilitator will assist with the identification of improvement opportunities; developing customized work processes for enhanced patient centered care by using principles of advanced access, panel management; mentoring and coaching of teams to implement best processes. This individual will be required to engage and develop relationships with EOPCN project teams and stakeholders to advance the medical home initiative, its advantages, and processes.
QUALIFICATIONS:
Please note that all candidates for the above position must have:
Experience:
- The ideal candidate will have a minimum of 5-10 years of progressive experience in a variety of leadership roles within the healthcare environment with primary care.
- Previous in-clinic, primary care experience is an asset.
- Previous experience as a coach, mentor or facilitator and experience working with family physicians and primary care teams is preferred.
- Experience, knowledge or additional training in change management, improvement methods and QI principles and their practical application as related to primary care is an asset (training in these areas will be provided).
- Experience in public speaking and presenting is desired.
Education:
- Undergraduate degree or diploma in a related field of study, preferably a health discipline. A combination of experience and education will be considered.
Special Knowledge/Skills:
- Intermediate to advanced Microsoft Office knowledge (Word, Excel, Outlook, Teams, and PowerPoint).
- Familiarity with the stages of change, system change theory, and how they apply to health care.
- Knowledge, skills, and experience facilitating groups
- Knowledge and experience working in Netcare
- Experience with multiple EMR platforms
License(s)/ Certification(s)/ Other:
- A valid driver’s license and access to a reliable vehicle
- Recent (within 3 months) criminal record check/police information check (including vulnerable sector search) prior to the first day of hire.
