The Quality Improvement (QI) Facilitator will advance high quality, patient-centered care across the Edmonton O-day’ min Primary Care Network (EOPCN). Reporting to the Program Manager (or designate), the role will support program managers and interdisciplinary teams in designing, implementing, and evaluating data-informed quality improvement initiatives.
The QI Facilitator will work collaboratively with clinicians, clinical support and administrative staff, and the Evaluation and Data team, guiding projects from priority setting and goal development to testing, refining, and monitoring changes using established QI methodologies.
The role will also build internal QI capacity by coaching leaders and staff, providing tools and education, and fostering an evidence-based culture of improvement. In partnership with the Evaluation and Data team, the QI Facilitator will ensure teams have the information needed to inform decisions, measure progress, and demonstrate outcomes aligned with project goals.

KEY RESPONSIBILITIES & DUTIES:
- Collaborate with program managers and interdisciplinary teams to plan, implement, evaluate, and sustain quality improvement (QI) projects aligned with EOPCN priorities.
- Coordinate and facilitate project meetings, supporting root-cause analysis, goal setting, identification of outcome measures, and development of change ideas using established QI and change management methods (e.g., PDSA, EPIQ, Lean Six Sigma–DMAIC, ADKAR, ARECCI).
- Support teams in creating and maintaining project charters, timelines, and progress tracking tools.
- Serve as a subject matter expert in QI and facilitation, applying proven tools such as SIPOC, process mapping, prioritization matrices, fishbone diagrams, and Gantt charts.
- Identify barriers to improvement and guide teams in developing practical, data-informed solutions.
- Work with Program Managers and the Evaluation and Data team to interpret data and translate findings into actionable insights.
- Assist teams in developing outcome measures, dashboards, and performance indicators.
- Support project sponsors and leads in planning, monitoring, and reporting on project progress and outcomes to inform decision making.
- Promote QI literacy among staff to strengthen understanding of improvement methods.
- Provide QI coaching, tools, and consultation to build capability among leaders and staff.
- Facilitate learning sessions and workshops on QI methods and best practices.
- Encourage collaboration across programs to share opportunities, lessons learned, and successful innovations.
- Foster a culture of continuous learning and improvement across all programs and teams.
QUALIFICATIONS:
- Bachelor’s degree or equivalent in a health related or social science field; QI training or certification (e.g., Lean, Six Sigma, IHI) is strongly preferred.
- Minimum 3 to 5 years of experience in healthcare quality improvement, program evaluation, or process redesign; experience in primary care or community health is an asset.
- Demonstrated experience facilitating or coaching multidisciplinary teams and interpreting data to support decision-making and improvement initiatives.
- Strong facilitation, coaching, analytical, and problem-solving skills, with proficiency in Microsoft Office and familiarity with QI or data visualization tools such as Excel, Power BI, and Visio.
- Excellent communication, collaboration, organization, and the ability to manage multiple projects with a high level of detail.
- Valid driver’s license and access to a reliable vehicle if community visits are required.
- A recent (within 3 months) Criminal record check with a vulnerable sector check is required.
